Below is an Affidavit of Residency for Enrollment in West Haven Public Schools for a student living with a relative or non-relative(5118/Form
No. 4). Once completed, please return the original affidavit to the
West Haven Board of Education, PO Box 26010 , West Haven, CT 06516.
This form must be signed in the presence of a notary
public. When using this form you must bring photo identification and two
additional documents from the list below bearing the name of the person
who signed above as the owner of the house or lessee of the apartment
in order to register.
Acceptable Documentation
- Copy of home mortgage payment
- Apartment lease showing name of the lessee
- Current utility bill (gas, electric, phone or water)
