Affidavit of Residency

Below is an Affidavit of Residency for Enrollment in West Haven Public Schools (5118/Form No. 4). Once completed, please return the original affidavit to the School Readiness Office, West Haven Board of Education, PO Box 26010 , West Haven, CT 06516.

Page 1:          To be completed by Parent and signed and dated in front of a notary public.

 Page 2:          To be completed by person(s) with whom you are living, also signing and dating in front of a notary public.

 Two (2) current proofs of residency must be provided by the person(s) with whom you are living:

•  Mortgage, rent, or lease receipt (must be one of the proofs)
• Court document
• Utility bill(s): UI, AT&T, RWA (copy of entire bill), SCG, Comcast, DirecTV, Dish Network, Oil
